User Accounts Are Not Getting Created When Effective End-Date Is In The Past

User Accounts are not getting created with error "User Account Cannot be created" in Manage User Account page.

The issue can be reproduced at will with the following steps:
1. Load Employee Data via HDL
2. Run post HDL Processes
3. Verify few Uses are not created with the error "User Account Cannot be created"

Validation steps:

1. There is no latest user creation request as part of PER_LDAP_REQUESTS table. There is only one request dated in the past which was faulted
2. The employee even though has active assignment, EffectiveEndDate is always: 05-MAY-14 which is in the past
