MDF Approval Status Does Not Change Once The Request Is Approved Or Rejected. (Stays "Submitted") (Doc ID 2132923.1)

Symptoms

We have configured one level approval workflow for MDF based on Country. This was working as expected till 24th April 2016.
But since then,we are noticing that even tough the notifications are being routed to the configured approver, once he clicks on Approve or Reject the status of the MDF is NOT changing accordingly. It stays in submitted status only.
This is blocking us from getting any MDF approved.

Please Note :Currently only first level has been enabled by us. All other levels in the BPM workflow have been disabled by clicking on Advanced-> Ignore Stage.

STEPS
1 - Create MDF Request with 1 User
2 - Submit for approval
3 - Log with Approval User
4 -Approval User Gets notification > Clicks on Notification>
5- Popup Opens that has the Request with Accept/Reject Buttons
6- User Goes to the Request Record and updates it
7 - Approval User Gets pag to the Popup with Accept/Reject and Accepts the Request
8 - Status of the Request does not change from submited
