Create Opportunity Results in Blank Screen (Doc ID 2139010.1)

Create Opportunity button does not work  

On the OSC UI, from the Opportunities tab, Summary, click on Create Opportunity, blank box appears.

When creating an opportunity in Classic, the following error appears:
 ORA-20001: No Document Sequence Assignment exists.  Details: A Document Sequence Assignment does not exist for Category Code MOO_OPTY_SEQUENCE_CATEGORY, Application 10032, Method Code A, Determinant Type {DETTYPE}, Determinant Value {DETVALUE}, and Transaction Date 2016-05-02.

SalesServer-diagnostic.log shows:

java.lang.Exception: An application error occurred. See the incident log for more information.
Caused by: oracle.jbo.JboException: JBO-29000: Unexpected exception caught: java.sql.SQLException, msg=ORA-20001: No Document Sequence Assignment exists. Details: A Document Sequence Assignment does not exist for Category Code MOO_OPTY_SEQUENCE_CATEGORY, Application 10032, Method Code A, Determinant Type {DETTYPE}, Determinant Value {DETVALUE}, and Transaction Date 2016-05-10.
