Global HR: Duplicate User Account In Sales Cloud System

After changing the role of a user account in our Federal Signal Sales Cloud system a second account appeared in the User Manager.  Now we cannot remove the old item as there is an error that appears in the Active area.  The Error states the "Username already Exists" however the account does not belong to the account that was modified.

"Username already exists."

The issue can be reproduced at will with the following steps:
1. Log into application
2. Go to Manage Users
3. Edit user
