Unable To Create SiteWand Account From Manage Accounts in SiteWand

Information in this document applies to any platform.
When logged into the server Manage Accounts screen, the user is trying to create new accounts. The user fills in the information next to Create (Account name, password twice, email twice, and time zone), then clicks on Create. After clicking Create, it just brings the user back to the login screen and nothing is created.
