ACTUAL BEHAVIOR

R11 Custom Object Lookup on Activity is returning a blank page

It seems to be unrelated to the Activity record or user, but happening for all Activities and all users.
When the user tries to associate a record from the Custom Object to an activity the Search and Select Custom Object screen is blank

EXPECTED BEHAVIOR
-----------------------
When the user tries to associate a Custom Object record to an activity the Search and Select Custom Object screen should allow for the user to perform the search and select a record

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login
2. Go to Edit Activity screen
3. Click on the DCL for Custom Object of a Task
4. Verify the Search and Select Custom Object screen is blank

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot associate a record from the custom object to an activity.
