Manager Hierarchies Not Updated in OIM

OIM is not syncing or updating after running processes. Manager hierarchies are not updating.
We are finding some users where the manager field in OIM is empty and should be populated. Or, after a change has been made to employee manager, the information is not reflected in OIM.
