Lead Conversion Is Doubling The Account And Lead Lines

On : version, Leads, Sales Campaigns

When converting a Lead, the Lead and Account are duplicated.

There should only be one Lead and Account after a Lead is converted.

The issue can be reproduced at will with the following steps:
1. Create a new Lead
2. Ensure that Account information is filled out.
3. Attempt to convert the Lead
4. Verify if a new Lead and Account were created, duplicating the existing Lead and Account.
