Symptoms

Duplicate user in Resource Directory and only one user appears in Manage Users

ERROR
-----------------------
Edit User page with User Details: The user-account request has failed. username XXXX already exists.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to Sales Cloud
2. Navigate to My Team > Manage Users
3. Search and Edit user XXX
4. Under User Details see the error: The user-account request has failed. username XXX already exists.

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot provision user roles.
