Error when adding new Organization on Manage Users page and cannot set the new Organization as the ROOT node in Organization Hierarchy

Symptoms

When attempting to add an Organization to a user in Manage Users the following error occurs:

"ORA-20105: Root Node exists. Multiple root nodes not allowed for this tree structure. ORA-06512: at "FUSION.FND_TREE_UTILS", line 1219 ORA-06512: at "FUSION.FND_TREE_UTILS", line 798 ORA-06512: at "FUSION.FND_TREE_UTILS", line 717 ORA-06512: at "FUSION.FND_TREE_UTILS", line 591 ORA-06512: at "FUSION.FND_TREE_UTILS", line 440 ORA-06512: at "FUSION.FND_TREE_UTILS", line 373 ORA-06512: at "FUSION.FND_TREE_UTILS", line 328 ORA-06512: at line 1"

Also the new Organization cannot be found under Organization Hierarchy in order to set it as the new ROOT node.

Changes

The new Organization created should be the Parent of all other Organizations and assigned to the new CEO.
