Creating new Account doesn't save the phone number

When we create a new account - there are 3 items that are always entered, Company name; Phone number; and City. The phone number is not being saved from the creation page to the details page.

It is expected that the phone number will save every time.

The issue can be reproduced at will with the following steps:
1. Go to Accounts
2. Select to create an account
3. Enter in required information, along with phone number
4. Click Save. Notice how the phone number does not save.

The issue has the following business impact:
Due to this issue, if the users do not realize that the phone number did not save, then they might not have the phone number to call the customer back with.
