SSO: The Assertion's Subject does not have a nameID (Error Reason 6)

Symptoms
After configuring for SSO, attempted zone access via SSO fails.
- The client receives error reason 6
AND
- An internal Taleo log contains an error about nameID missing from the Assertion's Subject:
Authentication Failed With Error: LOGIN_ERROR_INVALID_ASSERTION: 6-null
