Documents Cloud Service --Cannot Access or Create a Service Instance

Symptoms

When attempting to create a new Documents Cloud Service instance the create process fails. Moreover, the user is unable to connect to the service that had already been created.  The user receives the following error:  

"Sorry, Something Went Wrong. This service is currently unavailable. Try again later."
