Fusion Applications - Manage Approvals Displays Too Many Approvers

Symptoms
When initiating transactions which requires human approval, sometimes the button "Manage Approvals" in Fusion Applications user interface displays more lines than needed.
In the example below, the requisition is only going to be approved by two people but in "Manage Approvals", each approver is displayed four times. Also, auto approval steps are displayed with the label "Applications Development Framework Application Identity for ProcurementApplications" which can be confusing.
The issue can be reproduced at will, for requisitions workflow, with the following steps:
STEP 1: login to Fusion Applications
STEP 2: create a purchase requisition with more than one line
STEP 3: before submitting the requisition to approval, click on "Mange Apprrovals" button and see there are approvers listed more than once
