Convert Pending Worker produces error but transaction processes

Some users since the R11 upgrade are seeing a series of error messages when they convert Pending Workers to Employees.
The system displays a message the transaction was submitted, but over top of that appears a message that an application error has occurred. After the user clicks OK, another message appears with an ADF_FACES error.
The most recent was approximately 9:45 EDT; several others happened around 5:27 AM EDT, 5:17 AM EDT and 5:07 AM EDT.

The issue can be reproduced at will with the following steps:
1. Navigator -> New Hire
2. Convert Pending Worker and submit
3. See error

The issue has the following business impact:
Due to this issue, cannot convert pending worker.
