Fusion/Taleo: Default User Group Didn't Get Assigned to New User

The Fusion-Taleo Configuration Guide says that the "default user group is automatically assigned only when no user group was manually assigned during the user account creation process."
However it is found that for some new users the default user group didn't get assigned.
- Client have no other integration running(TCC or webservice) to create the users
- The user account's history doesn't show who or what created the user account.
- The Taleo implementation includes Onboarding (Transitions), and it includes a new hire / user process.
