The Workflow of Assessment Value is Triggered Twice

On : 031CS version, Web Services

When updating Assessment Record associated to Service Request, the workflow for object Assessment Value seems to be triggered twice.

The workflow should be triggered only once.

The issue can be reproduced at will with the following steps:
1. Open the detail page of Service Request.
2. Scroll down to the related information section Customer Satisfaction Survey.
3. Update a survey to trigger the workflow.
