ACTUAL BEHAVIOR

Opportunity Assessment - Certain Questions Fail to Display to End Users

System is Release 10 on Test environment.

Opportunity Assessment is automatically associated to Opportunities with "Project Pursuit" sales method
For one question group: "Products on Project" - questions exist in the Assessment definition, but fail to display to end users
Issue is reproducible for all users on all browsers (see steps below)

This Assessment was functioning correctly as early as last week. Last weekend a patch was applied to the Stage environment. Could this be related?

EXPECTED BEHAVIOR

Customer should be able to see Assessment on "Products on Project" question group (Screenshot #2)

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
Navigator > Setup & Maintenance
Manage Opportunity Assessment Templates
Drill down on the active Assessment Template
Verify active questions exist in Question Group (Screenshot #1)

Navigate out of Setup & Maintenance and to the Opportunity object
Create a new Opportunity using the appropriate Sales Method
Click the Assessment sub-tab
Click the "Load More Items" hyperlink to display the question and question group
Note that the questions do not appear in the "Products on Project" question group (Screenshot #2)
Note also that questions for other Question Groups do appear - both before and after this Question Group
Test the issue in multiple browsers: IE 11, Firefox 51.0.1 and Chrome 56.0.2924.87. Issue is reproducible
Clear cache, cookies, temp files etc. Re-boot computer. Issue is reproducible.

Inactivate the Assessment. Recreate the Assessment. Associate the new Assessment to the Opportunity Sales Method. Retest using all steps above.
Result: Issue is reproducible with new Assessment - same Question Group is affected
