Territories Are Not Being Correctly Assigned And the My Territories List Shows Unexpected Results

ACTUAL BEHAVIOR

Account Territory list filters
the Account Territory filters are not assigning the territories properly.
An user is assigned to a territory. When he queries within My Territory record set for an Account which is also assigned to this territory, there is no result. When the user queries with the record set My territory hierarchy, the Account is returned.
Territory: The user queries within the record set My territory for an Account. He gets back an Account, which is assigned to two territories to which the user is not assigned.
EXPECTED BEHAVIOR

The user should see the accounts within my Territory OOB saved search with the territories he is assigned to
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to account icon
2. select the My Territory search
3. Verify accounts are returned that do not have the territories the user is assigned to
BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users are getting results in the list that they should not see
