OIM User Creation Request Failed with Warning IDM System null,Unknown Failure (Doc ID 2250172.1)

Symptoms

OIM user who has same last name and first name with the existing user will not be created because user-account request failed
with warning null,Unknown-account Failure in IDM System during Add User Request. Contact your System Administrator.

Steps to reproduce:

1. Create person with HDL
2. Run ESS "Send Pending LDAP Requests" process
3. Normally even person who has same last name and first name with the existing user,
    OIM user will be created with unique common name by adding alphabet.
    Sometimes OIM user will not be created.
4. Check Manage User Account for the issue person
    Warning null,Unknown-account Failure in IDM System during Add User Request. Contact your System Administrator is appeared.
