Error When Opening Any Account

Symptoms

On : 11.12.1.1.0 version, Accounts, Contacts, Households

Rel12 Error when opening any account

This happened just after upgrading to Release 12. Any time an Account is tried to be opened, the following error occurs:
 

ERROR

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Go to Accounts
2. Search and find an account.
3. Try to open an account and see the error

BUSINESS IMPACT
-----------------------
The issue has the following business impact:
Due to this issue, users cannot get to accounts.

 Upgrade to R12
