STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Ran regular payroll process through
  a. Checklist
  b. Calculate Payroll
  c. Calculate Prepayment
  d. Archive (did for both a regular pay and supplemental pay)
  e. Make EFT payments
  f. Generate Checks
  g. Run Third Party Periodic Tax Extract
2. All payroll processes worked but the tax extract generated blank forms
  a. Noted Audit Detail has data
