Symptoms

On : 11.1.11.1.0 version, Data Quality, Customer Hub

Post Release 12 upgrade Accounts and Contacts cannot be created from UI due to an error.

ERROR

An application error occurred. See the incident log for more information.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to Sales Cloud
2. Navigate to SUI > Sales > Accounts
3. Click Create Account button
4. Enter required details and click Save and Close button
5. The error occurs

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, accounts cannot be created.
