R12 Custom Object Pages Blank, Cannot Set Security (Doc ID 2267692.1)

This pod was provisioned in R12

A custom object was created and added as a subtab of Account
When navigate to Account then click the Custom object tab the page landing page is blank
The custom object has page layouts that are configured and has many fields and also has been made visible on the spring board and the object icon is not visible
When navigating to Application Composer -> Custom Object -> Security there are not any options to set standard and customer role security 

Security is to be set on the custom object and custom object records created.

The issue can be reproduced at will with the following steps:
1. Create custom object
2. Create DCL pointing to Account -> Name
3. Added custom object to Account as sub tab
4. Open an Account
5. Click sub tab
6. Screen is blank
7. Go to application composer > Custom object > Security
8. Page does not show rows with roles names
9. There is only one row, Custom Objects Administration
10. After clicking the link in the row the customer object name_c is visible
11. Custom object security cannot be set 

The issue has the following business impact:
Due to this issue, users cannot create records for the new custom object.
