Converting Certain Leads Throws Error

When attempting to convert a Lead, the following error occurs.

The value of the attribute RelationshipId is not valid.

The issue can be reproduced at will with the following steps:
1. Open an existing Lead
2. Click on Actions > Convert
3. Follow on-screen instructions until you get the error.

The issue has the following business impact:
Due to this issue, users cannot consistently convert a Lead.
