OAC Service Creation Failure because of DBaaS No Space left on device

You are unable to create an instance for  OAC service - it has failed twice and has no explanation why in the log. The last message in the activity log is "Started operation to check provisioning status on the VMs for [BI]"

The issue has the following business impact:
Due to this issue, users cannot create instance
