Symptoms
Running RMAN backup to Tape or local disk can primarily fail with error:
RMAN-03009: failure of allocate command on dev_38 channel at 05/08/2017 16:12:30
ORA-17619: max number of processes using I/O slaves in a instance reached

and sometimes accompanied with error:
ORA-27192: skgfcls: sbtclose2 returned error - failed to close file
The error conveys that RMAN was unable to allocate more channels, to perform backup.
However there is enough CPU resource in the server to allocate channel and there are no other backups running.
