User Accounts Are Locked/Expired After R12 Upgrade (Doc ID 2285717.1)

Symptoms

On : 11.12.1.0.0 version, Technology Management - Fusion Security

ACTUAL BEHAVIOR
---------------
Few Users are unable to Login after upgrade to R12.
Found that these User Accounts are locked after the upgrade.
The POD is SSO Enabled and uses IDP Login directly, no Chooser Login available to users to Login to fusion locally.
Customer would like to know the root cause on why User Accounts are locked after the upgrade.

EXPECTED BEHAVIOR

Expect User to login successfully

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Upgrade Fusion from R11 to R12.
2. Login to Fusion as a User
3. Observe the Authentication failed error

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot Login to Fusion.
