Bank Details Are Not Appearing On Personal Payment Method Page (Doc ID 2286505.1)

Symptoms

On : Rel 11 version, UK Payroll

ACTUAL BEHAVIOR

Bank Details are not appearing on Personal Payment Method page.

We have few employees to whom payment method details are present in database but when we are looking for the details via fusion pages details are not appearing.

The issue does not occur for every record. We need to understand what exactly the issue is with the record and how can we resolve the issue. If we identified issue with back end data then should be able to recognize how it could happen.
Further, there is no list of values for bank and branch. It is free text for us and we can enter any value in it. Which in returns create hz_party records for bank and branches.
Same records are there in Test and same in PROD but we have issue only in PROD.

EXPECTED BEHAVIOR

Payment method Bank details already present in database to appear.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. trying to create/ select Bank Account (intermittent issue) under Manage Personal Payment Methods;
2. Bank details not appearing.
