Information in this document applies to any platform.
User initially receives message: Your account is locked. You can unlock your account by going to FORGOT PASSWORD. User follows the FORGOT PASSWORD process but does NOT enter the challenge questions (because the challenge questions/answers had not been originally set up).
The password reset process then fails with the following error due to the "challenge questions" not being set up:
IAM-3040002: User challenge questions have not been set
