Error 'IAM-3040002: User challenge questions have not been set' and the User's Account Is Locked but Does Not Show as Locked in the Cloud Admin Utility (Doc ID 2291625.1)

User initially receives the following message:

Your account is locked. You can unlock your account by going to Forgot Password.

If the user follows the Forgot Password process but does not enter the challenge questions (because the challenge questions/answers had not been originally set up), the password reset process then fails with the following error:

IAM-3040002: User challenge questions have not been set

If this same situation were to occur but the challenge questions were previously set, the user would be able to use Forgot Password process to reset the password successfully.
