Symptoms

 
After upgrade to R12, the log in sits to the far left of the sign in screen, however the user is successfully able to login

 

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Open R12 to sign in
2. UserId and Password are to the far left
