Unable To Access URL Attachment In The Email - Error 404--Not Found

When attempting to open an attachment link in a workflow email,
the following error occurs.
The issue can be reproduced at will with the following steps:
1. The setup is as follows:
MAX_NUMBER_OF_EMAIL_ATTACHMENTS : 50
MAX_SIZE_OF_EMAIL_ATTACHMENTS_IN_KB: 10000 (10MB)
2. For some requests, users receive a PDF attached to the e-mail and can view the attachment with no problem. For others there is no attachment, and when the user clicks on the “URL Attachment” link at the bottom of the e-mail, he gets the “Error 404” message found below after logging in to Fusion Applications.
