R12 Upgrade - Edit Account - Numerous Layouts & Column Label Changes

Symptoms

On : 11.12.1.0.0 version, Accounts, Contacts, Households

ACTUAL BEHAVIOR
---------------
R12 Upgrade - Edit Account - Numerous Layouts & Column Label Changes

The summary table column values for sub tabs Opportunities & Leads have completely changed after the upgrade to R12.

 - Contact Phone & Rep fields disappeared on the Edit Account Overview Page.
 - When editing a contact within the account, the column "State" had it's label change to "Primary Contact State".
 - When editing an account under the Contacts sub tab, the summary table column "Title" had it's label reset to "Job Title".

EXPECTED BEHAVIOR

Layouts shouldnt be changed after upgrade

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login to Sales Cloud
2. Navigate to Accounts and see how the layout looks like -> Different than Release 11
3. Navigate to Contacts and see how the layout looks like -> Different than Release 11

BUSINESS IMPACT

The issue has the following business impact:
Due to this issue, users cannot see same layouts and affects users usability to the system
