This document explains why quotes submitted to approval via a certain reason do not advance to the Approved step once they receive the required approval. In this case, Quote History XML was only showing these actions: Request Approval[Submit quote] and then Approve[Submit quote]. Submit [Submit quote] was also expected at the end of the process after all approvals were given, but this didn't appear in the XML because the action set was unable to complete.
