Post R12 Upgrade File Attachment Issue
We have a custom field 'Lessee' on Opportunity layouts which is a DCL to Accounts.
Added attachments are appearing on a different Account record than where they were added.

Attachments should not appear on other Account records to which they were not added.

The issue can be reproduced at will with the following steps:
1) From an Opportunity, if we click on the Account (Lessee), which will take us to the Account, and add an Attachment, Save--no problems here.
2) Now, if we open ANY other Opportunity, click on the Lessee, which will take us to a completely different Account, we will see the same Attachment saved to the other Account in step 1.
3) If we search from the Account side, the Attachment isn't attached as seen in steps 1 and 2.

The issue has the following business impact:
Due to this issue, attachments are not being restricted to the Accounts where they are saved.
