Error: "IAM-3040002: User challenge questions have not been set" and User Account is Locked But Does Not Show as Locked in Cloud Admin Utility (Doc ID 2339282.1)

After authenticating to SSO, you are redirected to an Identity Manager page which shows the following error:

Your account is locked. You can unlock your account by going to ....


If additionally clicking the "Forgot Password" link in the error page above, the reset password options fails because challenge questions were never set up for the user, resulting in the following error to occur:

IAM-3040002: User challenge questions have not been set.

The issue can be reproduced at will with the following steps:

  1. Connect to the Cloud URL
  2. At SSO page, enter your username / password.
  3. You will see redirection to Identity Manager page showing error: "Your account is locked. You can unlock your account by going to ". Click the "Forgot Password" link
  4. While completing the steps to reset the password, note the error which occurs.
