Users Not Receiving All Email Approval Notifications

Users say that they are not receiving (all of) their email approval notifications. All BPM Worklist notifications are received, but not all the Emails associated with these notifications are matched via Email.
The internal IT department states only some of email notifications reached the network. The IT technician requested additional information from Oracle and the following was found.
