Unable to create Customer Site -Too many matching records found error

We have an error while adding Site to the Existing Customer account. This is impacting users to add PO and create Invoice against that Site. Now we are in middle of submission of invoices but no luck.

Too many matching records found. Specify additional criteria to limit the number of records.

The issue can be reproduced at will with the following steps:
1. Steps: Billing> Manage Customers>Search Customers> Select Account>Add Site
