Oracle Access Manager Error

After Release 12 upgrade, users have started receiving the access manager error randomly.

System error. Please re-try your action. If you continue to get this error, please contact the Administrator.

The issue can be reproduced at will with the following steps:
1. Login > After some time the user gets this error message

The issue has the following business impact:
Due to this issue, user cannot login
