Symptoms

Requisition > Actions > Document History

Document History for specific Requisitions does not record Approved Actions.
Action Details shows that the requisition has been approved.


Steps to Reproduce:
Issue is sporadic and only occurs with few requisitions.
1. Go to requisition Requisition
2. Review Document History
Notice that it does not show Approve records for the day when the requisition was approved.
