Bell icon not executing Approve/Reject (Doc ID 2388214.1)

Symptoms

Customer created Workflow to Approve Reject a task in application composer using the business process ExtnBusinessProcessComposite.

When they click on Approve or Reject from the Bell icon it is not updating the account record's to approved or rejected.

It works fine when they drill down on the notification and approve or reject from there - just not the Bell icon. Notifications are working as expected.
