Error while Loading Benefit Enrollment

We are trying to load Benefit Enrollment through Spread Sheet Loader. We are getting below error for almost every employee.

However when we try to load data for just one employee out of the same batch with new batch name without changing the data it is getting loaded successfully.

ORA-20001: An error occurred. The procedure ben_bpi_shd.constraint_error failed at step 5.

STEPS
--------
The issue can be reproduced at will with the following steps:
1. Load Workbook spreadsheet for benefits for all records
2. Error is thrown for almost every employee
3. Take one of the employees failing with error and load for just that one employee
4. Record loads successfully.
