ADF FACES Error Accessing / Editing Leads After R13 Upgrade

Symptoms
On : 11.13.18.02.0 version, Leads, Sales Campaigns
When attempting to create a new lead / updating the lead, the following error occurs.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login into application
2. Navigate to Leads tab and click on create lead button
3. provide all the required information and click on save button
4. Above error pops-up
5. Open any existing lead
6. try modifying the data and click on save button
7. Above error pops-up

Due to this issue, users unable to create / update any leads
