Upgrading APEX on DBCS ERROR: Could not find any APEX files corresponding to version "version" inside of the known locations

When trying to upgrade APEX according the the Database Cloud Service (DBCS / DBaaS) documentation errors are seen in the ords.log:
2018-05-16 10:37:05 - ERROR: An error occurred while validating the APEX directory "/u01/app/oracle/product/apex/5.1.0.00.45" for version "5.1.0.00.45"

In this case the upgrade was from 5.0 to 5.1.0, however, the same issue could occur with other upgrades described in the following DBCS documentation which discusses the supported upgrade paths:
