Symptoms

When open the OEM "SQL monitor" screen and stay then the following error
occurs after 1 minute.

 ORA-31011: XML parsing failed
 ORA-19213: line 27
 LPX-00005: Entity ref Name buffer overflow, maximum size is 256 bytes
