Unable To Use A Code Combination Error ORA-20000: "kff_lock_combination" has failed (Doc ID 2444160.1)

Symptoms

On : 11.13.18.02.0 version, Set Up FCR-Define Ledgers

When attempting to use an account combination on the Create Chart of Accounts Balancing Rules, the system shows the error number 1. Trying to add the combination in Manage Account Combinations shows the error number 2

ERROR

1. This combination cannot be used; it either does not exist, or using it here is not permitted.
2. "ORA-20000: "kff_lock_combination" has failed. Application Id: "101", Key Flexfield Code: "GL#", Structure Instance Number: "2001", Data Set Number: "". ORA-06512: at "FUSION.FND_FLEX_PRIVATE", line 178 ORA-06512: at "FUSION.FND_FLEX_PRIVATE", line 4581 ORA-20000: Combination already exists 1 time(s). ORA-06512: at line 1"

STEPS

The issue can be reproduced at will with the following steps:
1. Create Chart of Accounts Balancing Rules
2. Enter the account combination
3. System shows error This combination cannot be used; it either does not exist, or using it here is not permitted.
4. Go to Manage Account Combinations
5. Try to create the combination.
6. The system shows error: "ORA-20000: "kff_lock_combination" has failed. Application Id: "101", Key Flexfield Code: "GL#", Structure Instance Number: "2001", Data Set Number: "".
