UCM Fails: Unable to connect to Content Server [FA_UCM_PROVISIONED] (Doc ID 2459451.1)

Symptoms

BI Publisher Report scheduling for UCM is failing.

While scheduling and selecting Output Destination Type as "Content Server" we get below error,

Unable to connect to Content Server [FA_UCM_PROVISIONED], please check server configuration




If we continue without selection, it still schedules the report. However it get below error,

Waiting Time: 1.2 seconds
Data Fetch Time: 0.2 seconds
Total Execution Time: 0.4 seconds
Job Priority: Normal

Delivery failed.
[INSTANCE_ID=bip.bi_server1] [OUTPUT_ID=18650]
Document delivery failed
[INSTANCE_ID=bip.bi_server1] [DELIVERY_ID=-1]Exception happened when delivery document to wcc
deliver API call throw ProcessingException

WCC delivery failed with Exception
WCC delivery failed with DeliveryException
oracle.xdo.service.delivery.DeliveryException: oracle.stellent.ridc.common.xml.XPathEvaluatorException: oracle.xml.parser.v2.XMLParseException; lineNumber: 23; columnNumber: 40; Namespace prefix ns1 used but not declared.
oracle.xdo.service.delivery.DeliveryException: oracle.xdo.service.delivery.DeliveryException: oracle.stellent.ridc.common.xml.XPathEv


This was working in the past.
We get same error for all the reports while scheduling to UCM.
And the same report work in other environments, so this is an environment specific issue.
