Invalid User Name or Password for All Users

Information in this document applies to any platform.
When attempting to login, all users are getting the following error:
Invalid user name or password
SEVERE: Unable to connect to the registry database.
java.sql.SQLException: Listener refused the connection with the following error:
ORA-12516, TNS:listener could not find available handler with matching protocol stack
1. Enter credentials to log in.
2. See pop up error.
3. Select ok.
4. Observe the program goes back to the login page.
Because of this issue, no user is able to work in P6.
