Audit Reports Not Working Correctly - Audit Records Created in Database But Not Visible In UI (Doc ID 2465545.1)

Symptoms

Admin User is facing some serious issues in terms where people claim that the activities are disappearing from the system.
Therefore, to trace this, the admin user enabled audit policies for certain objects in the system, e.g. Meeting Minutes.
Unfortunately, this is not working correctly, the audit records are not showing in Audit Reports.
Meeting Minutes is not showing what was modified, in Audit Reports for the Attribute.
