Cannot Rehire Employee in Production

Payroll Coordinator is trying to rehire employee.  Every time she tries to rehire the transaction fails with the following error message:

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. rehire employee
2. transaction fails

BUSINESS IMPACT
-----------------------
The New Work Relationship transaction was submitted last week before our 18B upgrade. We have now upgraded to 18B. Had the Payroll Coordinator delete all of the phone numbers instead of the Primary Work Phone on the old assignment of the employee but now the Manage Person Transaction has failed.
